Dissolve the instant coffee granules in 1 tablespoon of hot water until fully dissolved. Set aside.
Add the sweetened condensed milk, heavy cream, and whole milk to a blender.
Pour in the dissolved coffee mixture, cocoa powder, vanilla extract, and almond extract.
Blend on medium speed for approximately 30 seconds until smooth and well combined.
Transfer the creamer into a clean glass jar or bottle with a tight-fitting lid.
Refrigerate for at least 1 hour to allow flavors to meld together.
Shake well before each use. Serve 2 tablespoons per cup of coffee. Store in the refrigerator and use within 7–10 days.
